Been dealing with this issue for months... So frustrating. Exact same issues... only in Chrome and would go away temporarily with a chrome restart. I finally just solved it. I figured that it must be an extension conflict. In my case it was... it was the Techsmith Snagit extension. As soon as I disabled it my problems went away. I was so excited that I actually just made an account to share this with any other perplexed and sad users. Cheers.
