I'm creating a performance review form. I have a section where employees need to rate themselves on a scale of 1 - 5. (1 = Poor, 2 = Fair, 3 = Meets Expectations, 4 = Exceeds Expectations, 5 = Outstanding) I have radio buttons for each option so that they are only allowed to select one. However, when I go in, as the employee, to fill out the form and select the radio field I want, the selection doesn't save. Is there a way to make it so that the radio button selections are saved? Or is there a better option?
