I am using Adobe Acrobat Reader DC version 2017.012.20098

There is a number in the upper left hand corner of my document. It has a long number and says submitted electronically. What does the number reference? I am asking because I need to track down when I sent an email with the attached form. Can ths number help me?

You'll need to ask the person/institution that provided you with that file. It's not a built-in feature of the PDF format, so we can't possibly know what it might mean.
