New to captivate 9, get this blank text box on last quiz question

It looks like a Runtime dialog, but with some wrong styling. Are using the 'Submit All' feature for this quiz?

Exactly when does this box appear over your question slide? When you hit Submit? or when you click again AFTER submitting?
