pdf form at end of tutorial

I created a tutorial using Captivate 6. At the end I put a link to a pdf form (a certificate of completion), but I am having trouble printing and saving the form. I can fill out the form just fine, but if I try to save it when using chrome, the form appears blank when I view the pdf file later.

Any suggestions on how I can make this work?
