How do I left align the 3 lines below to the word "Is" on the first line?


I don't have any means to attach an image, but this is simple:
Set the left margin for a slight indent (start with 1/2 inch).
Set the first line for a matching "outdent" (negative indent, -1/2 inch).
Set a left tab at 1/2 inch. (Optional but good practice.)
Now, a tab after the number will line up the whole paragraph. It's all in setting that hanging outdent, which you can adjust to suit your style.
Be sure to create a paragraph style for this, and update it if you make changes.
