I created a new Text Variable, wihtout any documents open.

I can see it , if I go to Text Variables / Define.

But if I open a document and choose Text Variable / Insert - it does not show.

How do I get it to be accessible from the Insert menu?

Hmm, interesting this is exactly the version I tested on my MAC. Did you clear out the cache folders as well? Try deleting them manually, the paths are mentioned in the article I linked to

Works for me. Which version of InDesign are you using? What is the type of the variable that you define? Try trashing your preferences and see if it resolves the problem

I gave that a go, but it still won't show up in the Type / Text Variable / Insert Variable options
