Placing images - they only appear on second try

I'm using ID 2021 on an older Mac (can't update it). When I place an image into my document, the first time I try (command-D) the window appears, but the file I just added to the folder doesn't show up. If I close the window and try again, the image is there. This is a long document (475 pages so far) -- is that the cause of this issue?
