"Edit in photoshop" not detecting changes; LR stuck in editing session

1. Right click on photo and select "edit in photoshop"

2. Make changes in photoshop. Save file. Quit Photoshop entirely.

First observation: none of the changes I made showed up in Lightroom

Expected: changes made in Photoshop should show up in Lightroom

3. cmd+Q to quit lightroom

Second observation: Lightroom shows a modal "Photohsop editing in progress"

Expected: Lightroom should quit quietly - Photoshop is not even open anymore
