fillable forms

I attached the form I started with and also what it looks like fillable,
see it is missing some fields that I want fillable.

Hope you are doing well.

I'm using "prepare form" to turn a pdf into a fillable document. It has worked fine for most of the documents I converted but for one it is skipping several lines I need fillable, how do I make those fillable?

It could be PDF specific issue. It would be better if you shared the screenshot of what was explained above.

Or you can share the file itself, we will test it at our end.

Also, make sure that the document you are working with is not a scanned file, it should have real text for better results.

In addition to this, install the latest update of Acrobat. Open Acrobat>help>check for updates.
