I am having an issue with the image fields on a form. The images are not showing up. Critical

I have created a form in acrobat Pro DC that uses the image field for users to upload pictures. Here is my specific version of Acrobat:

Here is my problem: When the user clicks the image field, they are prompted to select an image file. The user selects the file, clicks ok, clicks ok again, and the pop-up disappears. The image is not displayed in the image field. After saving and re-opening the form, its as if the image was never selected. I need this to work with jpg, png, and any other image type. It seems as if this should be a straight forward field - select image and it is displayed. I searched the manuals and either missed the information I needed or it is not present.

Please advise. This is a critical time-sensitive issue.
