What product do I need to create a PDF form that allows users to upload a jpg image

I need to create a PDF form that allows users to upload a photo (jpg format) into a PDF form and then save it and print it. Right now, I have created one using a button script in Acrobat but it is not at all user-friendly because it only allows other PDFs to be uploaded on certain machines.

I have a sample PDF that allows the exact functionality that I'm looking for which allows all image files to be uploaded into the field, but I don't know how it was created. Is there an Adobe product that I need? I currently have Adobe Acrobet Pro DC on a Mac.

It was likely created using LiveCycle Designer, which allows this type of image field. Adobe currently only allows Reader to select an page from a PDF to be used as a button icon. There's hope that this may be changed in the future, but for now you'll have to use LiveCycle Designer: Download Livecycle designer tool | Adobe LiveCycle Enterprise Suite 4

but it's only available for Windows.
