Hi,
I created a version of a business form with Adobe Pro XI, however whenever I have the company complete the form and send it to their client for signature, all form fields disappear. Process is as follows:
Employee opens form in Adobe Reader app, completes
Opens file in Adobe Sign to send for signature
The files are available on Dropbox - Adobe I uploaded the form that I created and also the form that the client receives when sent from the app (file.png).
The original form that I created seemed to work fine, however we noticed that the signer was able to change the fields, so I went ahead and edited the form fields again in the version 2 doc as by sender.
Any advice to help me would be appreciated! Thank you!!

Do you have an example of a form where the form fields have disappeared? I;m not asking for one with real data, but rather a test with sample data entered.

Yes, you can see the example form in the dropbox link that I posted. The title is file.png. This is what the client sees when the quote it sent for signature.

Did the client use Acrobat Reader or Adobe Acrobat?

The client receives an e-mail from Adobe Sign to sign the document, which includes a link to view the proposal. Between when the company completes the proposal in Adobe Acrobat app to opening it in Adobe Sign...all form fields are lost.

I was able to figure this out on my own. When I created the form the fields were "Sender" instead of "Prefill". This seemed to fix the problem.

I am having the same problem. I don't see the form fields as Sender/prefill.
Can anyone help on this issue?

If you right click and look in Properties, then you can define who is to fill. The problem is that these Properties dissaapear, lock, and change constantly for no good reason. Not to mention, the fileds still dissappear with great frequency when the signer receives the document.
