I have 2 email addresses, one for any work/school related communications and one for all the other stuff.
I want to upgrade from the free version but before I do, I'd like to know if I can use one licence for both of my email addresses on my phone and on my computer or would I need to upgrade to Acrobat Teams?

As far as I know, your Adobe account is linked to a single email address registered to your account. The danger of using a work or school email address is that as you move on to your next job or finish studying, that email address is not accessible unlike a Gmail or Outlook.com (personal) for example.
Also, double-check, but you should be able to install the software on 2 devices using the same account. I use Acrobat on a tablet and a computer without issues.
You can run a trial for free and test it all before you buy.

Your subscription is entitled to your email address. One subscription/license can be accessed by only one email on which it is registered or purchased.

One subscription can be accessed from two different devices, but if you are willing to access your one subscription from 2 separate email addresses, that's practically impossible.
