Created an internal pdf accessible to all employees.  Employees fill out info and return form.  Does this violate the 500 copy limitation?

It only applied if you applied the Extended Reader Rights to the file, which are no longer needed.

No, sorry, you'll need to do your own research on that.

Clearly if you don't add the rights, an agreement about adding the rights won't apply. But you have to stop adding the rights (and retest).

so would be filling out the form.
