Acrobat Form Not Working Properly | Fill & Sign

I am trying to fill out a form in Adobe Acrobat Pro DC. 

Adobe tells me that the creator has made the form fillable, and I know that it has been successfuly used by many. 

I can check the check boxes with no problem, however I cannot type anything in the other form fields. 

I thought perhaps my keyboard was locked, but that is not the case. 

Any ideas?

Anyone else who runs into this issue--I was able to resolve by restarting the computer.

The form is private to my company, but I can see if I can find a picture on the internet that looks similar to what I am seeing.

This is just from an internet search, but the fill in boxes that I can't seem to type in look like the blue boxes towards the top, just without text inside. There are a couple of places that I can check yes or no, and those do work, just not the typing/text fields.

Thank you, however my problem is that nothing shows up when I click and type in those blue text fields. Permissions and settings are correct, but I can't type in them despite that.
