Acrobat Forms Fields disappear on Combine and Save As

When I 'combine' an Acrobat Form and a scanned PDF image - fields randomly dissappear from the form when I 'Save As' - grrr

We are sorry for the trouble. As described above the form fields disappear on combining the PDF image.
Would you mind sharing more information for a better understanding:
- What is the workflow or steps you are doing to combine the PDFs?
- What is the version of the application installed? To check the version of the application please use the link :https://helpx.adobe.com/acrobat/kb/identify-product-version.html
Make sure you have the latest version.Go to Help > Check for Updates - Is this a behavior with a particular PDF or with all the PDF you try to combine. Try to combine different PDFs and check.

OK - so when I view the combined doc up in the browser I can see all the fields.. But when I open it in Acrobat the fields are missing!!

A new version of the Adobe Acrobat /Reader dc got rolled out version 19.21.20058. Please update the application to the new version available go to Help > Check for Updates. and see if that helps

We are sorry for the delay in response. We tried to reproduce the issue at our end and its working fine.
Please make sure you have the latest version of Acrobat DC 19.21.20061. Go to Help > Check for Updates. You may also
manually download the patch using the link https://www.adobe.com/devnet-docs/acrobatetk/tools/ReleaseNotesDC/index.html
If it still doesn't work, please try repairing the installation (For Windows only) Go to Help > Repair Installation.
If it still doesn't work, please try the following steps:
- Remove the application using the Acrobat Cleaner Tool https://labs.adobe.com/downloads/acrobatcleaner.html
- Reboot the computer
- Reinstall the application using the link: https://helpx.adobe.com/download-install/kb/acrobat-downloads.html

I am on Version Acrobat DC 19.21.20061.33636
I repaired the installation - It still dosn't work.
I removed the application using the Acrobat Cleaner Tool
I rebooted
I reinstalled the application and rebooted.
It still doesn't work....
I sent you the 3 files so so can check them??

Thank you for trying the suggestions. As the problem still persists, please create a new test user profile with full admin rights and try using the application there and check for the issue.
If it still doesn't work, please use the procmon tool (https://docs.microsoft.com/en-us/sysinternals/downloads/procmon) to collect the logs. open the procmon tool and then reproduce the issue and collect the logs. Share the logs with us in private message for further testing. Please click on the message icon on the top right corner of the screen to start the private message.
