Acrobat Forms Fields disappear on Combine and Save As

I'm still having this issue.

I create a fillable PDF Form - save it - works fine.

Combine it with another PDF - (A blank page) - thi screates a BINDER - SAVE AS a new PDF File - It all looks fine UNTIL I Page up or SCROLL then ceratin fields disappear - always the same ones...

Any help would be appreciated - I am on the latest version etc etc...

After the save as a new file - if I then go back to PREPARE FORM - Acrobat can't find any fields and treat it like a new form...
