ACROBAT PRO IN PERSON SIGNATURE SOLUTION NEEDED

Hi, 

I recently started consulting in a hospital. I need to be able to fill out forms in front of the patient and then have them sign the form on my Ipad infront of me. I could not find an inperson form signature video. The patients do not have emails for me to send the forms to via email and have them click on the e-sign option. Is there a way for me to fill in the form asking the patient the form questions, then having them sign the form in the location they need to sign? this is the most important feature in my work flow. please advise.  Additionally, is there a way to set buttons to export specific pages in the long pdf form to print to pdf or save the specific pages in a specific order? please advise.
