Acrobat Pro X doesn't OCR all text

Acrobat Pro X stopped OCRing all text. It will OCR some of the text but not all of it.  I checked my scan settings and everything is the same as how it was last time it worked correctly scanning everything. I don't know if there was an update that messed with it or not.  All text still gets OCRed when I initiate the scan directly from the scanner into a PDF but if I initiate the scan within an already created PDF by using the "insert from scanner" function, not all the pages get OCRed.  This makes me believe it is an Adobe issue and not a scanner issue. Any ideas on how to get it to go back to OCRing all pages like it used to?

There is no recent update in Acrobat X, as it goes end of life in 2015. There is no difference between 'Create New PDF' or Insert option related to OCR. It might be issue woth document or settings.

I suggest you to make sure 'Recognize Text' checkbox is ON, when you are scanning the document.

Also try to scan same document from both Insert and new PDF options to ensure, document can be OCRed or not.
