Add form fields from templates in Acrobat Pro?

I use Acrobat Pro to create fillable forms with four different fillable header options (1-6 fillable fields) for adding contact information to the top of the form (see example image below).  I currently have the headers in separate files and copy and paste the fields onto each fillable form to create the form with each header variation.  It's really time consuming so I'm looking for a way to make this process more efficient and much faster.  Here's what I've tried so far that hasn't worked:

 

Hiding Fields

I tried adding all four header fields onto the main document (sitting on top of each other) so that I can hide the header fields that aren't needed.  But manually hiding each header field takes more time than cutting and pasting.

Layers

I tried to add the fillable header fields as layers but then they don't work as fillable fields.

Templates

I tried creating templates from the header files, but I can't find a way to add the header template to the main document in Acrobat Pro.  

 

Replace Page

I tried to add header fields to the form background (the text under the fillable fields) and then used the Replace Page function to change the main form background so it would have that header.  But the header's fillable fields didn't transfer.

 

Combine Files
I tried combining a header file with the main form file (and vice versa) but it didn't work.


Do you know of any way to do this?

A script can be used to add the fields to the file. You can create multiple variations of it to add different fields in different locations, with different settings, and assign each of these commands to a menu button so you could add them with a single click.
