Adobe Acrobat PDF Form Fields Error

Hey, I have one page PDF Fillable form, but I want to add 2 more pages and add Image upload fields on them. But when I add pages using Organize Pages tool and add Image fields on these pages, it does not appear in the Reset and Submit buttons. 

On clicking the Reset button, only previous form fields are reset but not new image fields.

What is the possible solution for this?

What does you use at the submit button?

Reset Form doesn't work on image fields anyway, since their icons are not considered values. You will need to use a script to clear them. Although, if the pages are spawned, you might want to delete them entirely when resetting the form, but that's up to you...
