When I am sending a document to be signed by someone and include a date field to be populated upon his/her signing, Adobe Sign is including multiple repeats of the date throughout the document in places where I did not include a field. Anyone know what this problem is or how to fix it? An example is included...

Please let us know how you created the PDF form. Are you sending the document using the Acrobat desktop application?
The screenshot shared above is not opening. Could you please share the screenshot again?

The PDF was created by combining documents downloaded from Clio Draft in PDF form and a Word document that was converted into the PDF. The multiple dates only show up on the pages from Clio Draft that are sent through Adobe Sign, not on the Word converted ones. The document is sent through my Adobe Pro desktop application. I'll try to include the screen capture again...

Sorry about the delay in response.
Thank you for sharing the screenshot and the information. It clearly seems to be an issue with the form itself. As the PDF was not created from Acrobat, there is not much we can do here without checking the original form. If you can share the form, we can replicate the behavior and share our findings.
If this happens with other forms as well, please let us know.
