Barcode is not getting visible in adobe reader

I made a editable pdf form with QR code in it which will contain the form data. But when I am opening the the same pdf in adobe reader QR code is not getting visible.

Could anyone please help me out to resolve this problem. I am attaching the pdf form as well.

This is not a problem, it is by design: Acrobat Reader don't display QRcodes.

It's generally cheaper and simpler to buy each person an Acrobat subscription, unless you have more than a few hundred users.
