I asked this in the InDesign forum, but narrowed it down to being an Acrobat issue:
I'm trying to create a ID form with 6 checkboxes. Problem is, when I export an interactive pdf, the checkboxes act like radio buttons ie, I can't check more than one box. I did the obvious and made sure each box has a unique name - they do - but I still have an issue. The form also has radio buttons, and those act as expected.
I thought a simple workaround would be to open the pdf in Acrobat DC and just add the check boxes there. Here's the weird thing, it happens in that program too. Even with different names, the boxes act like radio buttons in Acrobat. I've had other people open the pdf and the checkboxes behave correctly.
So far I've tried:
Rebooting
Clearing preferences
Exporting the file from CC 2015
Uninstalling and reinstalling Acrobat
I'm running the latest versions of Acrobat Pro DC and Indesign CC 2017on a PC.
Anyone have a clue what's going on?

Many will tell you it is best to do the form layout in InDesign but only add the form fields using Acrobat.
If check box fields have the same name property, then they are considered to be in an exclusionary group and each field must have a unique export value. The use of the unique export value allows the checking of individual check boxes. If the check boxes have the same name and export value then they are treated like the same field.
Check boxes with unique names are considered independent check boxes and work independently of each other.

If you re-read my post, you'll see the problem isn't that I didn't give the check boxes unique names. The problem is that I did and I still can't select more than one.
Likewise, the problem is the same if I create the form in Acrobat.
