Combining multiple pages of the same fillable form

I am trying to put together multiple pages of the same form that are fillable with signatures and dates in Adobe Pro DC. I saved each page as a separate document and when I combine them, only the first page has the fields for signatures and dates, all other pages the fields are gone.  Can someone help me with this?

As per your description, all the documents are filled and signed. Could you please confirm if the forms are digitally signed or via the Adobe Sign?

If yes, it is not possible to combine those documents. However, you can create a PDF Portfolio of all the documents.

You may use the steps to create a PDF portfolio as suggested here: https://helpx.adobe.com/acrobat/using/create-customize-pdf-portfolios.html 

 

If that is not the case, would you mind sharing the documents or the video of the issue occurring at our end?

Share the Acrobat and OS version on the machine. Also, let us know the steps you do to combine the documents.

The forms are not yet signed. The only thing needed on these forms is for different department heads to sign and date, then return to me. Each form is exactly the same, except the vessel name at the top of the form, but that's a text field not a fillable field.

I have also tried making a portfolio and it does the same thing. The first page looks like the above form, page 2 to the end is this form without the fields. They are gone.

I'm not sure what I did differently before, but when I created the portfolio this time, it worked!

We are glad to hear that it is working fine for you now.  A PDF Portfolio contains multiple files assembled into an integrated PDF unit. The files in a PDF Portfolio can be in a wide range of file types created in different applications.

 

The original files retain their individual identities but are assembled into one PDF Portfolio file. You can open, read, edit, and format each component file independently of the other component files in the PDF Portfolio.

 

Depending on the circumstances, PDF Portfolios offer several advantages over merging multiple files into a single PDF. For more information about PDF Portfolio please look at the help page https://helpx.adobe.com/in/acrobat/using/overview-pdf-portfolios.html
