We have a fillable form with an embedded email return address. When returned, the data fields are blank (they are all required). We have tested internally and had no problem, but when sent back by the client it returned blank. Clicking into the field did not cause the text to appear BTW.
This is the instruction we send to the user:
I have attached our client intake form. This form just helps us collect as much information as we can in one location. It is a password-protected document. You will receive another email with the password.
Please be sure to fill in all the required areas then hit the green button on the bottom right-hand side to send back directly to me password protected.
What are we doing wrong????? Thanks!

What tool are the recipients using to fill the form?
Do you know the exact steps of how the users are accessing the form?
Are they filling it on a desktop system? a mobile device?
Details are important. Find out the exact process the recipients are using.

Add something like this to your instructions:
You must save the file to your local computer and then open it using Adobe Reader only! Opening it in any other application, including in a browser window, will cause the file to malfunction.
If you don't have Adobe Reader you can download it for free from here: https://get.adobe.com/reader/

We send fillable forms to 3000 employees. Most of them don't have Adobe Reader nor will they download it. So now what?
