Coverting CMYK Values to Spot

Hi all.

I'm trying to create a Preflight Profile that converts specific CMYK values to specific Pantone spot colours. I've done this successfully before for different colours/clients, but I'm running into trouble with this one.

Here are screenshots showing the output preview hovering over the CMYK orange and blue in question:

Firstly, when I try to edit the ‘Covert to spot color’ fixup, I get this error:

But if I switch to List View, it works, and I set this up:

So I OK that, save the fixup and profile, and try running the profile, but it doesn't run that particular fixup. Here's the completed window:

It's run the other two fixups in the profile (the normalize ones), but not the spot conversions.

Does anyone know what might be going wrong, or how I could better go about this?

I'm using Acrobat DC 17.009.20044, under OS X 10.12.4

Danny,

Under no circumstance should you be getting any asserts while running any part of Acrobat including Preflight.

Look in your e-mail for a message from me giving particulars on how to send me the relevant files and custom profile and fixups such that we can debug them here at Adobe.
