Create a simple ajax request

Hello everyone, im trying to create a simple http/ajax request.
It should works like this:
when you click on imge (i've prepared form, and added execute JavaScript code) it will send a simple http/ajax request to server. Its a simple get, and i want to see in server console that my request works, thats all.

Can someone just show me a code how to do that, because im working with that for a days, and nothink is working.
