Using Windows 10 v.1903

PDF is 336 pages
comments start on page 159.
First tried to create summary using letter size it cut off the page on the left, so changed it to A4 now it's cutting off the top of the document.
The pages before it are landscape, if I try to create summary from page 154 to 202, it always creates the summary starting on page 159 and it cuts off the tops of all the pages from 152 to 202, but didn't even add the pages 154-159
Tried exporting the pages and adding them back in, no change
Tried exporting the pages and saving them as jpgs and adding them back in, no change
tried deleted the pages and then importing them back in, no change, any page that has comments on it, its cutting off like the top 3-4 inches of the pages.
If I try to create the summary for the whole document it skips all the pages ahead and goes right to page 159, it is a scanned document merged into the document, yet later on in the doc there are other scanned images and they summarize fine and not cutting off the top of the page.
Any ideas welcomed
