Create form to sign that allows jpg attachments or other pdf

Estou utilizando o Adobe DC Pro e não estou conseguindo fazer um formulario para Assinatura que contenha a possibilidade do assinante anexar arquivos como jpg ou pdf.

Ja li artigos aqui: https://helpx.adobe.com/br/sign/using/create-forms-with-acrobat.html, mas não consigo colocar em prática.

Estou usando um formulario criado no Word, e convertido para formulario Pdf, mas quando incluo campo tipo Imagem, fica salvo, no entanto ao enviar para assinatura o campo não fica disponivel para a pessoa que ira assinar.

Minha ideia é automatizar um processo de propostas de socios ao quadro social de um clube, mas preciso de informaçoes e copias de documentos. 

Eu li no link acima que os documentos anexados seriam incorporados ao pdf assinado, seria uma otima opcao.

Could you please share your workflow to send the document for signature?

You seem to be converting the PDF form to an Acrobat Sign document. If that is the case, it is why users do not get the Image field. 

When you convert the form, it does not include the image field. It is not possible to add the image field in the form using the current Acrobat Sign plan.

Mas não encontrei uma explicação mais clara de como solicitar anexos durante junto de um documento assinado.

Sorry for the delay in response.

 

The option to add the attachment field is available in Acrobat Sign Team, Business, and Enterprise plan.

We have checked that you are using the Acrobat Pro DC subscription, which includes the Acrobat Sign Individual plan. The option for the attachment field is not available with this plan.
