Hello,
I wanted to see if there was a way in Adobe or using Javascript to create a button on a fillable pdf form that would give the user the option to add a second page to the form/report?
Example:
A user of the fillable form "X" can select an "add a page" button that would be invisible once submitted or printed. When the user would select this button, they would be able to choose from a small list of 2 or 3 different forms ("Y", "Z", or "AA" that they could use as page 2. They could then have the ability to add pages 3, 4, 5, etc. if they needed to from the list given.
If it can't be done on Adobe, if anyone knows where it could be done, or the best way to accomplish this, that would be great! Any help would be greatly appreciated! Thanks!

It can be done, but will require quite an advanced setup, using hidden Template objects and scripts to spawn copies from them, as well as a custom Print button added to the file.
