Creating a PDF as a non-standard page size

I'm trying to create a PDF by printing a Word document to a PDF.  The Word doc has a non-standard page size (19.42” w x 12.5”h).  Under Adobe PDF Document Properties/Adobe PDF Settings, I’ve set both the Default Settings and the Adobe PDF Page Size to match the desired page size. Still, the PDF creates as an 8.5 x 11 portrait document.

How do I get the PDF to create in the size I want it?
