Creating academic notes from ONLY the pdf comments

I am trying to export the comments and highlights from pdf books I am using for academic study.  I want to only export the comments and highlights as text into a MS word file (or something I can easily and quickly reference) with the page number, and the highlighted text or the comment.  i intend to use these notes when writing as references for citations.  

i am using the latest CC acrobat and to this point i have not found an acceptable solution.

There's an option to create a comments summary from the Print dialog:

However, the output is not really efficient, as the comments from each page will start on a new page, and the structure of the report can't be customized. To do that you would need to use a script to generate it, like this (paid-for) tool I've developed: http://try67.blogspot.com/2008/11/acrobat-create-comments-summary-txt-pdf.html
