Adding on to Test_Screen_Name...

Specifically, set up the source document (such as Word) in landscape orientation so that it creates a landscape PDF automatically when exported.

 

Try not to rotate the page once the PDF is created because there are known bugs and hitches that happen when PDF pages are rotated.

++ Adding to the suggestions provided by Test_Screen_Name and Bevi_Chagnon___PubCom

 

If you are creating a new PDF form in Adobe Acrobat DC you may try the following steps and see if that helps:

 

1. Go to Tools > Prepare Form > Create New > Start

 

2. By default the page orientation is set to portrait, now go to Tools >  Organize Pages > Rotate the page to Landscape orientation. click close on the tool bar to come out of the Organize pages tool.

 

3. Go to Tools > Prepare form > Single File > Save the file with name to proceed. Now you will get a landscape page with prepare form options  to add different form felds as shown in the screenshot below.
