do i need to continue my subscription for continued access to pdf form?

I'm looking at purchasing a subscription to create a pdf form that is interactive. I'd need it to be signed, dated, and several checkbox questions to be answered and sent back to me for it to fulfil it's purpose. I'm wondering - if i create this document, do i then need to keep my subscription in order to continually use this form? Or once it's created I no longer need the subscription?

How is it to be sent to you exactly? Adobe's send and distribute services need a continuing subscription in some cases. Plain email can work, but is not recommended for business it serious use as it is unreliable and insecure. Some kinds of signing may need subscription. Consider HTML forms.
