Document Cloud, Acrobat form fields with ghost values

Whenever I try to use an Acrobat form on Document Cloud, all fields retain ghost images of any default/previous values (see attached image). This renders them unreadable after changing any values. This occurs when accessing Acrobat forms thru my account and when accessing thru a shared link. The ghost images disappear after resizing the page, expanding/collapsing the sidebar, or opening a downloaded version (see attached image). It's not so bad for me, since I know I can simply toggle the side bar to eliminate the ghost values. But, I can't expect people I share it with to do this.

Also, possibly related, I get a popup error (see attached image) after changing any values and pausing for about 5 seconds.
