Duplicate bookmarks using Acrobat Word to PDF add-in

I have been working on a Word document of about 6-7 pages that I was saving as a PDF using the Acrobat PDF add-in for Word. The Word document has 9 level 1 headings. The first 5 have at least one level heading under them. I asked the add-in to convert those to bookmarks in the PDF.

 

Yesterday, I happened to notice that the bookmarks in the PDF are not right. The forst 2 level 1 headings are correct, but then the PDF has three more exact copies of the second level 1 heading and all of its level 2 headings.

 

I cannot figure out what is going on. I checked for updates and did install an update. That did not fix the error.

 

I cannot attach the actual documents due to private content, but I did edit it to remove it. I will try to attach the  edited Word document and the resulting PDF with the duplicate bookmarks.
