So I try to create/edit an editable form using the Edit Form feature which uses dropdowns and other javascript usage. But then after filling up the editable entries of the form and requesting for e-signatures, it messes up the format, the text, sometimes removes the entries. It's like a total different form.
As an alternative I tried to Print it first, then request for e-signature the printed PDF. This works fine. However, is there any other way to skip this extra step and keep the format of the edit form and go directly to the e-signature portion without messing it up?

Basically, I created a PDF Contract/Agreement that has a Fill-up page where you can just input all the needed info and it distributes it throughout the contract/agreement, instead of going to each detail/info one-by-one. To further describe the pictures attached:
- rep1: Shows the Editable Form that I made using "Prepare a Form" feature along with a few javascript codes.
- rep2: Shows that when I fill up the Editable Form, it then adds the information/details throughout the whole document/contract to make it easier in filling up the contract/document.
- rep3: After all the info has been filled-up, I use the "Request for E-signature" feature but then it changes/removes the Editable Form that I made earlier
- rep4: It also disrupts the information filled throughout the contract.
What I want to know is if there is any way for the E-signature feature from changing/disrupting/removing the editable form that I made initially? Thank you.
