Editing a pdf where several fields automatically fill in from another field and Auto complete is OFF

We are having an issue with random fields on this pdf filling in from another field, or when one field is answered it unclicks the answer in another field. In a 3 pg document this is happening in about 5 places.

I have no idea how to edit these fields to keep this from happening or why this is happening. 

The Auto complete is turned OFF.

Did you check if those fields have same name or check if there is any script that causing it?

Sounds like radio buttons that are part of same group.
