Esignature no longer showing when document sent

Hi, everything was working great - I was sending out T&C's for signature feeling very professional then I sent another set of T&C's and even though it showed the fields for the Name, signature, and date when the document arrived with the client there was nothing there. Not very professional and despite trying everything suggested nothing is working and I am close to canceling the trial period, which ends on 18th October.
