Extraer texto subrayado de un pdf

Alguien sabe cómo extraer el texto subrayado para en dos clicks lo convierta en un nuevo texto (resumen de lo que considero de un tema de la facultad)

It might not be possible at all, since the underline is often a stand-alone graphic element, not a part of the actual text.

Does the text appear as underlined when you copy it from the file to Word, for example? If not, it will be extremely complicated to do it, and not possible at all from within Acrobat itself.
