Fill and Sign not working

I have an adobe PDF form that i made from a work document. 
Some of the boxes can be checked off in adobe using fill and sign but some of the boxes cannot.

Please confirm if you have added any form fields to the PDF. It seems on one side, the form fields are applied, and on the other, it is just added images.

Would you mind sharing the PDF form with us so we can check it?
