Fillable order form

I'm having great difficulty trying to figure out how to create forms (that I originally created years ago in Acro Pro). I have attached a sample with this post. When I created these, the program gave me a number of item (i.e., date, image box, etc) that I could drag & drop anywhere on the page and align as I wanted. I also had tables that would total quantities and pricing. This could be opened and filled by anyone in our company. I need to recreate these forms with new logo, address, etc. Where do I start?

The file already have fillable fields, and it's protected, so I'm not sure what exactly you need help with?

I want to create a similar form from scratch. I did these years ago in what was then Acrobat Pro. I have no idea what program I would use now. The program back then had libraries of the various elements (i.e., date field, image box, text box, tables, etc.) that you could click and drag into the document. You could then drag the to where you wanted them and size the with the cornor toggles when selected.
