When I print a fillable form I have created it is now printing with the check boxes filled in. The form was created in InDesign and the .pdf was made into a fillable document with Acrobat Pro. This file has always worked in the past however today it started to print with every box filled in. It seems to only be happening on my computer, my coworker was able to print it correctly. This is happening for all of the forms I have tested so far today.

It turns out that my printer driver was bad. I was able to install a new driver and it works perfectly now.

The actual form is in my original post. What prints out is in my reply.

OK. I don't have the issue. Can you test another fillable PDF to rule out whether the issue is specific to this form? Also, in the page view in your print dialog, does it display the check boxes this way?

Yes, this seems to be happening to all of my fillable forms and on my computer only. The page view in my print dialog does not show them filled in.
