Fillable text issue in Adobe Forms

I make forms often for work and have been getting feedback from my team that this one form is not filling correctly and they can't always read the responses. Any ideas on how to fix this?

Make sure there aren't duplicate fields (with the same name) one on top of another.

If that's not the case then find out what viewer is being used for filling in the form when this happens. Ask for a full-window screenshot.

Double-checked and there aren't duplicate fields. I did change the font from helvetice to arial in case they don't have that font on their computers. I'm assuming since not everyone has adobe programs on their computers most are filling this out in an internet browser.

Here's the original form attached. I only have screenshots of the form not performing correctly (had to crop to hide personal information). When my team messes around with it it always fills correctly.

Some fields are not properly aligned, but otherwise the form is correct.
As try67 assumed, the problem lies with the user and his software.

Thanks for the insight, any recommendations for what I should suggest to the users to correct this? Are better ways to set up a file you know users will be filling out in an internet browser or on their phones?
