filling out application and adobe is adding my name to all the name blanks

Hello, I am trying to fill out a scholarship application that is a pdf file.  When I put my name in the field that says name it is adding it to every name field.  There is a place that has to be filled out by a teacher and if I delete my name and deletes it from all the name fields.  I hope this makes sense to someone and I can find the answer to fix it.

If all fields are named the same, they will all have the same value. The person who created the file probably thought it will be easier for users to don't have to write their name multiple times.
