Formulario pdf enviado por correo

Hola, estoy creando un formulario,  que automatice un instrumento,  sumando puntaje , tengo 2 prwgintas: 

La primera:

Como puedo dar al valor total mediante un check , ejemplo de cada check de q punto me da un resultado y este resultado  tenga según un rango de valor ejemplo 10 a 15 puntos da un letra , ejemplo  C1

 

La segunda: 

Como configuro para que ese formulario se envíe por correo y me de una base de datos para trabajar.

Agradezco información, orientación a quein pueda cooperante
