Generate bookmarks for a PDF created from Google docs

When you save a Google doc file as a PDF, it doesn't take the table of contents to create the bookmarks in the PDF. Is there already a tool that does this? If no, could I write a plugin that could do this?

It might be possible using a custom-made script, depending on the structure of the TOC in question.
