Help with yes no circle

I have a form document and need to  remark the N with red circle or the Y with read circle ;

 

need instruction and screen shot how to do that including the code script if yo can

This can be done in many different ways, one of the easiest would be to use radio buttons.
Create two radio buttons with different names (for example: Group1 and Group2, so they are not mutually exclusive) and put them over letters Y and N,
1. Change 'Line Style' to Solid (see image bellow).
2. Change border color to 'No color'.
3. Use this code in both radio buttons->Properties->Action->Mouse Enter and select 'Run a JavaScript':

event.target.borderColor = (color.equal(color.transparent, event.target.borderColor)) ? color.red : color.transparent;


Now when you put mouse over letters it will give you red circle or if you put mouse again it will remove circle.
